- If it’s been more than two weeks since your last injection, you’ll need to restart at the initial dose.
- If you’ve continued receiving this medication from another provider during your break, you must send proof to care@joinlevity.com before placing your order.
Proof of Treatment Requirements:
Please provide a photo of your current prescription label, ensuring that the following details are clearly visible:
- Your name
- Prescription date
- Medication name
- Dose and strength
- Date of your last injection
Important: If we don’t receive confirmation that you’ve continued treatment elsewhere, our clinical team will restart you at the initial dose.
